Whether you are coming in for an annual physical, a specialized screening, or a routine check-up, our dedicated team of Family Nurse Practitioners takes the time to listen and understand your unique health history. We help you navigate the complexities of preventative medicine so you can focus on living your life with confidence and peace of mind.

Wellness Exams

A wellness exam is a yearly visit focused on maintaining your overall health, preventing illness, and promoting a healthy lifestyle. Unlike visits for specific conditions, this appointment gives you and your provider the opportunity to review your health in a comprehensive way—helping you stay on track with preventive care and early detection.

  • During your wellness exam, your provider will:
  • Review your medical history and current health status
  • Perform a review of systems, discussing how each part of your body has been functioning
  • Check vital signs, including blood pressure, heart rate, weight, and BMI
  • Order preventive lab work to monitor cholesterol, blood sugar, thyroid function, and other important health markers
  • Update recommended vaccinations
  • Discuss healthy lifestyle habits, such as diet, physical activity, stress management, and sleep
  • Recommend age-appropriate screenings, such as mammograms, colonoscopies, or bone density testing

These visits are an opportunity to partner with your provider, set wellness goals, and create a proactive plan to support long-term health.

Women’s Wellness

For women, a wellness exam includes additional screenings and conversations focused on reproductive and hormonal health. These may include:

  • Breast Exam: To check for lumps, changes, or abnormalities that may need further testing.
  • Pelvic Exam and Pap Smear: To screen for cervical cancer and detect early cellular changes.
  • Mammogram Orders: Based on your age and personal or family history, your provider may recommend and order a mammogram.
  • Colon Cancer Screening: Recommendations for colonoscopy or stool-based testing depending on your age and risk factors.
  • Reproductive and Hormonal Health: Discussion of menstrual cycles, contraception, fertility, menopause, and sexual health.

Our goal is to provide a comfortable, supportive environment where women can openly discuss their health and receive the preventive care they deserve.

Men’s Wellness

Men benefit from annual wellness exams that emphasize prevention, early detection, and overall well-being. These visits are designed to help you stay ahead of potential health concerns and maintain peak performance at every stage of life.

A men’s wellness visit may include:

  • Comprehensive Physical Exam: Assessment of heart, lungs, abdomen, skin, and other systems.
  • Preventive Lab Work: Evaluation of cholesterol, blood sugar, thyroid function, testosterone levels, and other health markers.
  • Prostate Health: Discussion of prostate cancer screening, including PSA testing when appropriate based on age and risk factors.
  • Colon Cancer Screening: Guidance on when to begin colonoscopy or other screening options.
  • Heart and Metabolic Health: Blood pressure monitoring, cardiovascular risk assessment, and lifestyle counseling for weight management and nutrition.
  • Lifestyle and Mental Health: Support for stress management, sleep quality, mood changes, and sexual health.

Our providers focus on building a trusted relationship with each patient, encouraging open communication and preventive care for lifelong wellness.

Medicare Wellness & Risk Assessment

For patients with Medicare, we offer Medicare Annual Wellness Visits (AWVs) and Health Risk Assessments (HRAs) designed to help you stay healthy, independent, and informed about your care. These visits differ from a traditional physical exam—they focus on preventive planning, risk identification, and coordination of care rather than addressing new medical problems.

During your Medicare Wellness Visit, your provider will:

  • Review your medical and family history
  • Assess your current health status and preventive needs
  • Complete a personalized health risk assessment (HRA) to identify factors that may affect your well-being
  • Update your list of medications and providers
  • Assess vital signs and key health indicators
  • Review your cognitive function and risk for depression or memory concerns
  • Discuss fall risks, home safety, and mobility
  • Update recommended immunizations and cancer screenings
  • Develop a personalized preventive care plan, including follow-up recommendations and health goals for the coming year

These visits are covered by Medicare once every 12 months and are an excellent opportunity to ensure all aspects of your health are being monitored and managed proactively.
